6th Grade Physicals:
As per state law, all students in the 6th grade must have a physical completed. Physicals dated on or after January 1st of the student's fifth grade year will be accepted. Physicals must be on file in the nurses' office by May 1st during the student’s 6th grade school year. The 6th grade physical must be completed on the State of Connecticut blue form. Elementary Schools Medical Update:
A medical update form is sent home in September. During the school year, please keep the nurse updated on your child's current medical issues. We also appreciate having current phone numbers available in order to contact you.
Medications:
Students are not allowed to carry any type of medication. Students who need to have medications in the nurse’s office are required to have the proper forms on file with the nurse. For further information and to download forms, please go to Health Services.
New Immunization Requirements for Preschool, Kindergarten and Grade 7
The Connecticut State Legislature is expected to pass additional immunization requirements for entrance into Preschool, Kindergarten and seventh (7th) grade effective for the start of the 2010 school year. The new immunizations are as follows:
For entrance into Preschool (in addition to current mandates):
Pneumococcal conjugate (1 dose for children less than 5 years of age)
For entrance into Kindergarten (in addition to current mandates):
Varicella 2nd dose
Mumps 2nd dose
Rubella 2nd dose
Pneumococcal conjugate (1 dose for children less than 5 years of age)
For entrance into 7th grade (in addition to current mandates):
Tdap (1 dose)
Varicella 2nd dose
Mumps 2nd dose
Rubella 2nd dose
Please be sure your child’s physician updates this vaccine(s) before the start of school.
Please note students will not be allowed to start school if written proof of the vaccine(s) is not provided to the nurse.
